Not having been a creature prone to sensation/feeling for eons, falling from grace was the first time Cas had become subject to physical sensations like pain, hunger, discomfort, suffering - and other more mundane sensations. The show in later seasons describes feeling as being something very overwhelming for angels who still have weakened grace left. The Cas we meet at his canon point has taken to unhealthy coping mechanisms to handle all that overwhelms him, which does include painkillers, weed and other drugs, alcohol etc.

Cas has willingly marched into a suicide mission he knew he wouldn't survive, on the orders of the only person he has left in the world. His sense of purpose and self-worth is shot to hell, and he defines himself very much by his remaining use - or lack thereof.

Cas is a fallen angel, and the world in which he exists is suffering from an apocalypse invoked by Lucifer and his demon, with Heaven having abandoned the world to its own devices, and God being confirmed absent and uncaring. As such, Cas' faith is shaken to non-existent, and he's jaded to blasphemous and dismissive about the topic of religion.

The world Cas hails from has been infected with a demon-engineered virus that has infected most people and was created to eradicate humanity. Given the curent state of the world, this feels like it requires a CW.

Another coping mechanism, Cas engages in frequent 'debauchery' as he calls it, staging orgies as a way to connect and find some pleasure before death. While he will certainly not flirt with any character he meets, nor jump into bed with any character available, he may express borderline unhealthy views about his own relation to sex.
